  1. Country, state, province, or region involved.
  2. Policy type.
  3. Date of loss.
  4. Date reported.
  5. Reason in the denial or reservation letter.
  6. Policy clause or exclusion cited.
  7. Evidence already submitted.
  8. Whether the insurer issued a final position.
